Step 1: Talk to Your Service Coordinator
Let your Service Coordinator know you'd like Radiant Home Care as your provider. You can also call us, and we'll help you figure out who your Service Coordinator is. -
Step 2: Authorization
Your Service Coordinator arranges your authorized services and hours with your plan. -
Step 3: In-Home Visit
We visit your home to get to know you and build your care plan around your authorized services. -
Step 4: Caregiver Match
We match you with a trained caregiver who fits your needs. -
Step 5: Care Begins
Your caregiver starts on the schedule in your plan, and we handle the billing.
Care doesn't stop at the first visit. We check in with clients and families regularly and update care plans as needs change.
